Reporting Unlawful Conduct

IP Fabric s.r.o. (“IP Fabric CZ”) is founded on respect for compliance with legal regulations and high ethical and moral standards. Unlawful conduct is unacceptable to us. To help prevent such conduct, we are prepared to receive your reports. For uncovering potential unlawful conduct, we have established an internal reporting system in accordance with Act No. 171/2023 Sb., on the Protection of Whistleblowers (“Act”), which is based on confidentiality and impartiality in assessing every report.

Who can submit a report through the internal reporting system?

IP Fabric CZ employees (including job applicants and former employees) and members of its management and statutory bodies can submit a report through the internal reporting system.

What is the purpose of the internal reporting system?

Whistleblowers can use the internal reporting system to report suspected violations of legal regulations specified in the Act.

For reports and whistleblowers as defined by the Act, the rights arising from the Act are guaranteed, in particular the protection of their identity and protection against retaliatory measures. All reports are handled confidentially and impartially, and if you remain in contact with us, you will be informed about the outcome of the investigation.

How to make a report through the internal reporting system?
Written and oral
Use the reporting form available on the website: https://ipfabric.whistlelink.com/ (“Web Platform”).
In-person reports
IP Fabric has appointed external designated persons with experience in whistleblowing and internal investigations from FairData Professionals a.s., registered office at Na Florenci 1332/23, 110 00 Prague 1, Company ID: 061 49 961. 

The designated persons are Lívia Djukić and Anna Sergejko. You can contact them by phone at +420 226 288 363 or by email at [email protected] to arrange a personal meeting for submitting a personal report.

External reporting system
Reports as defined by the Act may also be submitted through the external reporting system established by the Ministry of Justice of the Czech Republic, see: https://oznamovatel.justice.cz.

How will personal data be handled?

Personal data is processed in accordance with personal data protection legislation. We keep records of received reports to the extent of: (a) the date the report was received; (b) the whistleblower’s name, surname, date of birth, and contact address, if known; (c) a summary of the report’s content and identification of the person against whom the report is directed, if their identity is known; (d) the date of completion of the assessment of the report’s merit or the assessment by the designated person and the result of the assessment. Received reports are stored for 5 years from their receipt.

In the case of reports submitted via the internal reporting system, only the designated persons have access to the records.
